Senior's Programs

Home 2 Stay Program

– The Home 2 Stay program assists individuals 55 years of age and older who are at risk of homelessness remain housed or find alternative safe, sustainable housing.

– Home 2 Stay provides intensive case management that works to stabilize clients’ housing by carefully assessing their risks and strengths, developing an individualized plan, monitoring progress, and making effective referrals to internal and community supports, advocacy and support with tribunal issues, and rapid rehousing.

– Home 2 Stay follows individuals for 12-24 months to ensure they are successful in maintaining their housing.

Crisis Intervention Program

– The Crisis Intervention Program supports individuals 55 years of age and older who are living in shelter/or has an urgent/emergent situation that requires immediate supports.

– The Crisis Intervention Program develops goal oriented crisis support plans aimed to assist individuals in stabilizing their current crisis and connects individuals with the most appropriate community services. A plan of care is created that responds to the presented crisis.

– The Crisis Intervention Program provides short-term case management (up to 3 months), as well as linkages and referrals to long term community, supports.

Senior’s Drop-In Support Centre

– The Senior’s Drop-In Centre provides a safe, informal space for individuals 55 years of age and older to gather.

– The Centre provides breakfast and lunch Monday through Friday, as well as access to case management with emphasis on low-income seniors who are vulnerable, potentially isolated or are living in at-risk conditions.

– The Centre develops programs of assistance for clients including referrals to internal services at St. Matthew’s House or external organizations or agencies that provide financial assistance, legal aid, housing, medical treatment, and other services.

– Social/recreation activities are also offered through the Centre.

Social Isolation Program

– The Senior Isolation Program is designed specifically to provide support for our isolated seniors 65+ in the lower city and east end through intensive case management and workshops.

– Through this program, isolated seniors will be identified and connected with a Senior Isolation Support Worker where they will work together to enhance basic life skills to improve their quality of life.

– The Senior Isolation Support Worker will work alongside agency/community partners currently working with clients to ensure that all needs are assessed and met.

– Social and Educational workshops will be offered within a social setting, providing isolated seniors with a safe environment to interact with other seniors in the community.

– Isolation amongst seniors is very common, it also looks different for each client.
